The first week my husband was in Rio Vista Post Acute and Rehabilitation was good. He received good care. The 2nd week they moved him to the 2nd floor. They appeared to be short staffed as it was hard to get him the attention he needed when he needed it. They only got him out of bed once a day for brief rehabilitation. They insisted in keeping him in bed where he grew weaker by the day. They kept him in diapers and too often waited and hour and half to get cleaned. Rooms are old...the whole facility needs an uplift. The nursing staff were very nice to my loved one. Jason in PT was amazing! This rating has nothing to do with the CNAs or Jason. Management is a joke! You have a lazy Case Manager (Harry) who spends more time on personal phone calls then he should returning calls made to him and/or messages left for him. No one EVER answers the phone at the front desk...I'm sure they screen their calls. They DID NOT provide proper PT/OT assignment at discharge and won't …

I went to Rio Vista for post surgery to have IV administered and therapy. Was not happy at first with the care provided until I got use to their program. Very happy with both Ernie & Jason, excellent care. The nursing staff was professional with a few exceptions.

My husband spent one night there. When he arrived the nurse did not come in to get to know him, but she did seem caring. The nurse the next day was not at all pleasant, to say the least. The beds were old and uncomfortable and there were holes in the walls. One bedside table was so damaged it would not be possible to clean it. The bathroom was old and dirty and completely wrong for a patient in a wheelchair. The toilet was tiny and the seat was loose, the floor was filthy. The food was awful. I got him out of there…

Understaffed nurses, management will not call back. Left message after message and no one cares. Food is Terrible. Nursing staff will leave patients for hours, understaffed and overworked. Physical therapy needs not met. Doctor doesn't show up half the time for patients. Will NOT Recommend this facility!!!

My mother is in Rio Vista Post Acute & Rehabilitation for rehab. I noticed that the people that work there are very friendly, very professional. The facility is immaculate and it's very clean. It's an older facility, probably, but it's very clean. I'm impressed with the health care workers there. She's going through physical therapy five days for one hour a day. She has a roommate, so she's sharing the room with another.
