My mother is here, but is leaving, she has always got a UTI, and is severely dehydrated. She is always hungry and so thirsty when I visit. And she is always dirty and smells bad. I am so disappointed. When she first went there a few years ago she could do for herself and was doing great. But now that she is completely dependent on others for everything, she's severely neglected. I never see her CNA, she's always without anything to drink. It's just a nightmare. Sad...

My mother is there and has fallen so many times. And constantly has a UTI. She isn't being cared for as I had hoped. The staff is never around. I rarely see the nursing staff at all when I visit. The call lights rarely ever get answered. I'm afraid she will get hurt really bad one day. I've notified the proper authorities. The area she lives in always smells like urine so bad. Im so heart broken.

My husband was in rehab at Bonifay Rehab. He was supposed to be in rehab, like have physical therapy and all of that, but he fell twice. He was only there briefly and nobody was really watching him in the middle of the night. They didn't have alarms in their beds, so my husband climbed up and fell again on the floor naked, crawling to his roommate. Then later, he fell again in the bathroom and had a seizure, that's why he went to the emergency. They're very negligent. I will never put my husband back…

I have been here almost 5 years. It was a huge adjustment as I came here when I was 49. Its not easy to keep someone like me busy and entertained but I am. unfortunately transportation to outside activity's is limited to walmart trips only. But the staff is really great. I have never in my life felt more loved and cared for. This place is always clean, well lite. Food is usually something that I can find a option. And I am a very picky eater. My Mom is in this Facility. She is 85 yrs old. She has been dropped many times on the floor. She fell out of her chair cuz the didn’t have seat belt on. No one was watching her. She hit the floor face first and split her eye brow wide open. Had to go to the hospital for stitches. This last episode, she went back to the hospital, dehydrated and with a UTI infection cuz they make her sit in her poo. I wouldn’t send my dog to this facility. Jus my opinion

This could be a better place but poor management.Never enough help.Patients are not cared for as they should.Linens and all laundry is always bad.Lack of supervision for employees .Patients health and well being are at risk.I have asked for help with no luck
