PositiveWhen my father was originally admitted to CC&RC, the staff seemed overworked and understaffed but they worked as hard and diligently as possible. I had a meeting with the Community Manager and Director of Nursing to express my concerns about the level of care provided to my father. The team listened and made necessary corrections. From that time onward, there were no issues with my father's care. Everyone on the staff from the kitchen all the way to the top is caring and professional. They took excellent c…

CriticalAt some times of the day, this facility seems normal except for the pile of smokers right outside the front door (County Health People, are you listening?) Smokers are residents, staff, and visitors. Then, evening came. The environment seemed to suddenly shift to One Flew Out of the Cuckoo's Nest. Literally, one 25ish patient swung around the corner with his leg extended forward on his wheelchair going at least 15-20 MPH, almost knocking the nurse coming in the swinging doors down. He appeared to be racing ano…

Criticalnot enough staff. brought in other staff from another care center only when state inspectors made a visit. all gone after inspection was over.not all staff observed the current covis guidelines. unable to get in touch with the social worker ,never got a call returned from her. I think she may of had a second job . when my father passed the staff released his body to the WRONG mortuary. Called the administrator about the mix up. he has yet to call back. Do not put a love one in this place. RUN someplace else

PositiveCommunity Care & Rehabilitation Center have short-term and long-term care. What I didn't like about them was the way they're going to finance the move of my mother. Mom would have been given $35 a month, and all her income will be taken as a share of cost during her stay. Also, she would have to live in a bed instead of her lift chair which she didn't prefer. For somebody who was fit, but in need of rehab care, it was an excellent facility. There were things I didn't like how they interacted…

CriticalMy father is currently 1/2 way through a 1 month stay for wound care and continuous IV antibiotics, otherwise he lives/drives etc independently. He's been very impressed with the day staff, evening staff is hit or miss... the NOC crew not so much, he has actually refused one RN to touch him. I am really concerned for the residents who are unable and/or have no one to advocate for them. I made a list below, but the straw that broke the camel's back was when his PICC line accidentally got pulled out about 8…

CriticalMedical care seems good. Nurses are hit and miss. Some are great and some are not. You have to walk through smokers to get to the front door. The only place to take a resident out to get fresh air is on the patio in the front which is filled with residents smoking. The entry even smells of smoke sometime. Poor maitanance. Wall paper is coming off the walls in the rooms. Resident cup is cracked badley and looks like it hasn't been changed in a week. Plugs are worn through and taped together and then the tape is…
