PositiveCottonwood Creek is an amazing facility that feels more like home than anything else. The staff and residents that live here are warm and welcoming and become instant family as soon as you meet them. Also, you can't beat the food. Please come and meet the family.

PositiveStaff/care is excellent. Everyone at the facility is very welcoming from the desk staff to the ED. This was a major part of our decision as it really feels like a warm, welcoming and engaged community. Activities are great and ever changing based on residents living in the facility. The activities Director is seemingly always there and creates programming specific to residents which is a big help with engagement. Food is great and the restaurant-style service is a really nice touch and makes residents feel specia…

PositiveThe staff has been great to work with, including [Name Removed] (who provides tours), [Name Removed] at the front desk,[Name Removed] the nurse, and[Name Removed], the activities coordinator. Everyone has been friendly, helpful, and attentive, and [Name Removed] in particular brings a lot of enthusiasm to the activities. My dad is in the smallest studio, which works well for him since he can’t walk. The room was clean when he arrived and included a brand-new TV and refrigerator. He describes the facility as beauti…

PositiveThe administration and office staff respond to questions in a very timely manner. My mother loves the food, and the rooms are beautiful, modern, and thoughtfully laid out. From the very beginning, the team was patient and supportive while our family worked through the decision for my mother to leave home. They truly reached out and made the transition feel comfortable. The nursing care is very organized, and communication with the medical providers in the facility has been excellent. We feel very fortunate.

PositiveThe tour was very informative. I was so impressed with Cottonwood. It has a happy, welcoming atmosphere. And Ethan was very kind as well as knowledgeable.

PositiveHad a good vibe. People I talked to were friendly and I got a good feeling. I heard the food was delicious there and they left me with some delicious cookies. I would send a loved one here.

Classification and identity evidence
Current Utah DHHS-licensed Assisted Living Facility Type I or Type II with 3 through 16 licensed beds and a non-expired license. Utah codes classify Type I/II facilities with 2-5 residents as limited capacity and 6-16 as small; this adapter conservatively retains 3-16 as a small-home proxy. The state does not certify the marketing label Board & Care, and the registry does not prove current availability, price, ownership, or an operator relationship.
