My mother initially came to Four Chaplains as a rehab patient. When it became apparent that she could no longer live independently, we asked her where she wanted to go, and she picked Four Chaplains on her own after resisting to go anywhere for years. The staff is wonderful and if there ever is a need or question, they are quick to address it. There are a great variety of activities which my mom really enjoys. The facility is always clean. My mother is in Four Chaplains Nursing Care Center. All of the nursing staff, including the nursing director, have been very responsive and helpful in terms of just going through all of this. It's been good. The staff is caring. The food was great. They have bingo, a movie and popcorn, and little exercise classes. Their physical therapy room is pretty nice. They have a barber shop or a salon, but I have not seen it. They have a courtyard, but they don't have anything beautiful outside, just some chairs. G…

I would never recommend this place. My mother came for a post hospital physical therapy. She saw therapy maybe twice was told the PT/OT room was closed due to Covid. Why would the accept anyone if services could not be provided? Was then discharged with wrong medical info and has not been able to receive continuing at home care due to this issue. Mother is still unable to walk properly. When called to express the situation was basically told, Oh well shes discharged now. A two week stay has turned into a total nigh…

I was a rehab patient here. I cant walk or take care of myself since surgery. They have great rehab here, why i was there. On friday at 3pm i was notified my insurance Aetna would not pay after saturday. I was told if my doctor says i need to stay insurance should ok it..cant reach anyone on the weekend. I was told by Deb/administrator that if i wanted to stay i had to pay up front 305.00 by check cash or card,im disabled and cant afford that. I asked deb,if i dont pay up front are you kicking me out,she said YES..…
