CriticalTo be honest the rooms were nasty my husband's bed sheets wouldn't even changed he was there for less than a month and the whole time we went and seen him the bed sheets were not changed at all and then one time I come in and he was laying in a fetal position cold because the room was freezing and his Peg to feed was out and the bed sheets were wet horribly bad two days later they end up with pneumonia and in the hospital and still in the hospital

CriticalI am mortified at the service provided to my grandfather during his stay at Oak Forest. He was there for 10 days before receiving a shower. PositiveWe had someone at Oak Forest Rehab. The workers were very good and very helpful. The patients were well taken care of. It's a very nice facility. They had rehab, physical therapy, a place where you can get your hair cut and washed, and they take care of the laundry.

PositiveStaff worked very hard to care for my mother who had behaviors and hurt the staff on a daily basis due to her dementia. They never complained; they just did their best to provide great care and compassion to her and the family

I enjoyed seeing the staff laugh and joke with my father, it really made a difference! His room was always clean and the building was always free of smells... that really impressed me. There are big windows in …

PositiveMy mom has been a long-term care patient at Oak Forest for many years now. We honestly could not ask for better care. She loves the staff and they have really become like family to us. There is no other place I would want her to be!
