CriticalHorrible care facility! My brother was placed there after a stroke, he was left in dirty diapers for hours. Patients were left in wheelchairs in halls and rooms unsupervised. My brother was found on floor after falling out of chair. We noticed his progress and health was declining, until he got so sick he had to be transported back to hospital. He had 570 blood sugar levels, high sodium level due to severe dehydration, sepsis due to UTI Infection, he could no longer walk or speak by the time they called emergency s…

PositiveOak River Rehab was a good community. It's both a rehab center and a nursing home. They had good quality care. They had a special place for rehab and exercise rooms. The staff was very helpful, caring about their patients, and gave them the quality rehab that they needed to where it helped get them out of there fast. The healthcare workers and the nurses made sure that the patients got the proper care so they can rehabilitate at home. The place smelled clean, too. It was a clean place. A lot of the rooms were …

CriticalMy pastor was at this facility for 10 days. He returned today, with nothing but the same dirty clothes he went in from the hospital with. The family took him 4 sets of brand new clothes and new shoes. None of which was sent home! They sent him home in someone else's old shoes, socks that were too big and not his! They didn't even send his comb home with him! I am applaud at this place! Please do not send your loved one here. Their 4 star rating is a lie! I am so tired of these medical institutions using c…

PositiveStaff It is not a superlative when I say that the staff, everyone of them, are the most genuine and caring healthcare professionals I have ever encountered. I am very astute when it comes to human character and I encountered not a single 'fake' person. There exists an upbeat, yet realistic approach to interaction with their charges. Staff reacts immediately to clients' needs as well as questions or needs family may have. If ever I need such care as they provide, I have no doubt that I would want to b…
