CriticalI truly wouldn't even give this facility 1 star. Very misleading place. Patient's are not heard and barely seen. Staff acts like you're bothering them when you need them. My mom was doing decent until she was placed here for rehab after a hospital stay. Her health decreased drastically after 10 days. She was transferred from there back to hospital. Everyone was shocked when she passed away a day and half after leaving this facility. PLEASE investigate and really look elsewhere before taking a loved o…

CriticalI’m giving this facility 1 stars, and those 1 stars are ONLY because of the CNAs and LPNs who have taken care of my loved one. They have been amazing and should not be blamed for the failures happening above them. The administration and everyone above the LPN level, however, have been an absolute nightmare. COMMUNICATION IS TERRIBLE. ACCOUNTABILITY IS NONEXISTENT. I have called repeatedly and repeatedly been sent to voicemail or told, “Someone will call you back.” I’m still waiting on calls that were supposed to…

CriticalThis place is budget to say the least. If you have a loved one that you want to be cared for and treated with high priority this is NOT the place for them. Please seek out other facilities before using this one. The staff is not knowledgeable at all and provide little care. The food is not editable and the nurse practitioner on staff should not even be licensed. Please don't mistake this Carmel Facility as being one of high caliber as IT IS NOT!!!
★★★★★
CriticalPlease do NOT take your loved one to this facility, The staff is uneducated and lazy. They don't provide proper care at ALL. Please also don't mistake a CARMEL address for being of great quality and care as it IS NOT!!! This facility should be shut down.

CriticalRooms are ok, staff is in short supply. Asking for help and waiting for over 30 min is not acceptable. This happened many times to my friend. She is on a restricted diet and her menu was never right, even after talking to the kitchen manager. Place seemed clean. Friend said she would never want to return here unless she knew it was better staffed. She needed more help and some CNA's told her they would have to come back later since they were working alone.

CriticalMy Mother has been here for almost 2 years and I keep hoping it will get better but it doesn't. I talk with her several times a week and see her at least once a week. The culture of the facility is secrets, lies and no confidentiality. The food is always cold and every 4-5 months they have a new chef. The activities director is different every 3-4 months. There is a new Business Mgr., Director of Nursing and Social Services Assistant during the time my Mother has been there. Some days there has only been a nur…
