CriticalMy mother was at this facility for the longest week of my life. The way they deal with Alzheimer’s patients is to ignore them. There were days that she refused her meds and they were fine with it. I would go there and she would have no water in her room. Her pajamas mysteriously disappeared and she slept in her clothes. I had to hire an outside caregiver to be another set of eyes. The place also smelled so bad and the nurses seemed like they were bothered if you asked for assistance. Do not send your loved ones the…

CriticalMy mom was at Sage Creek Post-Acute for rehab. The place was clean, as far as we know. My dad had stayed there before as well, but because of COVID we were never able to actually go in the building and see, and we were not able see her at all. The way they transferred her from there to the other community was really bad. They did not send her medications with her. We were promised a wheelchair; they did not send that. There was really bad communication. We had called several times to try to get a hold of her case w…

CriticalMy mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy. Everyone else was to busy in meetings. Because of COVD-19 I could not visit her and had entrust the health care workers to take care of her. The Facility missed her Doctors Appointment and displaced the …
