I visited this facility

I went to Care Partners Assisted Living in Hortonville. The setting is beautiful. There are plants and fountains out the windows, and there are patios. It's in a very, very nice area with lots of land. It's not like in the neighborhood. It's got land around it. The director was thorough in her tour, totally knowledgeable, and answered all of our questions. When I called her back since then, she answered all my questions. They have a craft room. Roomy and compassionate

The memory care units are spacious. Large enough to accommodate a twin bed, nightstand, dresser, two living room chairs and end table, small two-chair dining, small refrigerator and TV stand.

Would recommend Care Partners

I would give it the same rating except a 5 for value. My sister would probably do the same. The activity rating wasn't given because Ed didn't do activities. Several times I saw them doing group activities and the residents seemed engaged and having a good time. I would put a solid 4 down for that. I would recommend Care Partners Hortonville to others.

Home Away from Home

My husband has lived in Memory Care at Care Partners in Hortonville the past three months. I am impressed with the kindness, patience, and expertise these Caregivers show in their respectful care of residents with dementia. The Activity Director is especially attuned to individual needs and abilities, even in a roomful of residents - a very talented self-esteem builder! All staff have been most friendly, even fun, giving this very attractive place it's homey feel.

I am looking to move my 90 year old mother from assisted living into a memory care facility due to her paranoid/dementia getting worse. Friday, 3/30/18 at 10:31 I placed a call to Care Partners and set up an appointment for Monday, 4/2 at 10:00 to visit the Hortonville, Wi facility. I gave them my name and my telephone number and was told at that time that they would see us on Monday. We drove 1-1/2 hrs from the south side of Milwaukee to keep this appt. It is hard to get a good feel because the facility is new and is only half full. There was not much activity for it being mid morning. The setting on the edge of Hortonville is quite appealing, quiet and peaceful. [name removed] was quite helpful and was able to answer all of our questions. We would not rule this facility out, and are likely to go at meal time and chat with some of the residents.
