We moved mom to Sage Meadows after a less than satisfactory experience with another facility. I highly praise the level of care, the amazing staff especially the Activity Director! Long term care management requires dedication, professionalism, great communication, and tenderness. The entire staff (including dietary and housekeeping and all other) excel in these areas. A great home away from home

It is very clean. The staff is friendly. Having a PT on site is great. The dining room is nice meals are ok. My loved one does not participate in the activities but it is not because of staff. They try really hard to get him to participate. Only thing I could say they could improve would be some communication. I think that is common everywhere. I would definitely recommend to others.

Our family had an in person tour and it was just not a good experience the community smelled and was quite dirty

My parents have moved into Sage Meadow of De Pere Assisted Living and Memory Care. It's a very caring, clean, and an excellent community. They have friendly staff. The food is excellent. They have bingo, exercise, and therapies (physical and occupational).

A very clean and welcoming facility all furniture, pictures and paint has been recently updated. All on one floor which is nice for residence large wide hallways.
